As of October 2025 Harmony no longer accepts Insurance.

Membership Service

Your chosen Harmony HRT membership includes:

• Provider visits and comprehensive hormone treatment & management

• Required blood labs as determined by your medical team and outlined in your package

  • Any additional labs you request beyond the package are billed separately

• Unlimited communication with our medical team

• One complimentary 20-minute visit (telehealth or in-person) per year for special circumstances (This is over and above what is included in the membership)

  • Additional visits beyond what is included in the membership plan: $60 each, due at scheduling

• Medical procedures are billed separately

• Family/Partner Discount: When family members or partners sign up together, each receives a 10% discount on their package

Payment Options

• Pay in Full

• 4-Payment “Pay as You Go” Plan

• Monthly Auto-Pay: Your card on file will be charged once per month for 12 months

• Debit/Credit Card

• HSA/FSA Card

• CareCredit (available for “Pay as You Go” option only)

Insurance & Billing Acknowledgement

As of October 6, 2025, Harmony HRT and its providers are not enrolled with any insurance plans, including Medicare or Medicaid. All services are self-pay only.

Please note:

• Claims for Harmony HRT services are not covered by Medicaid or Medicare, and will not be submitted for reimbursement.

• Harmony HRT cannot bill any insurance programs.

• Patients who choose to receive care with Harmony HRT agree to pay out of pocket for their package or membership.

• Full financial responsibility rests with the patient for the selected membership or package.

• No refunds are issued once payment has been made.

• Prescriptions and referrals will continue to be provided when medically appropriate; however, your insurance plan may not cover these because our providers are not credentialed with insurance companies.

• Financial Assistance

• We understand that medical care can be a financial burden for some patients.
